Admissions Forms

Thank you for your interest in enrolling your child at Covenant Christian Academy. Please follow these three simple steps to print out the correct application forms for your child.

Step 1

Each family should print out the document below and fill out the appropriate information one time regardless of the number of children you are enrolling:

Next, please print out the following grade specific Application Packet for each child you are enrolling and fill out all of the forms that are included:

Pre-K through Grade 1
Grade 2 through Grade 4
Grade 5 through Grade 6
Grade 7 through Grade 12

Step 3

Please return completed application in person, mail, fax, or scanned e-mail attachment to:

We are still accepting applications for the 2010-2011 school year as space is available.  Application forms may be downloaded and printed or picked up in the Admissions Office.
